Provider Partners Health Plan Signs New Partnership with Keswick in Baltimore

LINTHICUM, MDProvider Partners Health Plan (PPHP) today announced a new affiliation with Keswick, a not-for-profit organization in Baltimore that provides a variety of services dedicated to enhancing the quality of life for older adults. PPHP will provide its Institutional Medicare Advantage Special Needs Plan (I-SNP) to Keswick’s long-term care residents.

PPHP offers long-term care residents an integrated and personalized approach to care that is managed and coordinated by a local provider clinical team—including nurse practitioners and nurse care coordinators who check in daily with members—thereby providing diversified care management enabling the team to address burgeoning health issues which ultimately reduces hospitalization rates for the member—and the facility.

To date, PPHP has enrolled more than 1,000 members in 22 long-term care facilities in Maryland and Pennsylvania. PPHP will begin enrolling members in November at Keswick’s 175-bed longterm care facility in Baltimore.

Keswick was first chartered by the Maryland General Assembly 135 years ago to provide services to senior citizens.  Today Keswick offers adult day care programs; post-hospital rehabilitation services; community health services such as chronic disease management programs, fitness classes, wellness coaching, and longterm care.
