Provider Partners Health Plan Signs New Partnership with Lorien Health Services

LINTHICUM, MD–Provider Partners Health Plan (PPHP) this month announced a new affiliation with Lorien Health Services, a family-owned organization that operates nine skilled nursing facilities in four counties in Maryland, including Baltimore, Carroll, Harford & Howard counties. PPHP will provide its Institutional Medicare Advantage Special Needs Plan (I-SNP) to Lorien’s long-term care residents.

PPHP offers long-term care residents an integrated and personalized approach to care that is managed and coordinated by a local provider clinical team—including nurse practitioners and nurse care coordinators who check in daily with members—thereby providing better care management enabling the team to address burgeoning health issues which ultimately reduces hospitalization rates for the member —and the facility.

To date, PPHP has enrolled more than 1,000 members in 22 long-term care facilities in Maryland and Pennsylvania.

Lorien Health, founded in 1977 by Mary and Nicholas Mangione Sr., provides a variety of services and therapies to seniors in long-term care facilities in Maryland. The company combines patient-centered care with the latest in healthcare technology.

PPHP began enrolling members in July at Lorien Columbia in Howard County and will expand to other Maryland facilities throughout the remainder of the year.
